We write to inform all NSPs in the municipality that, the annual evaluation forms are now available on our dashboards.
*Personnel are required to print it out and submit to their organizations of serving to be endorsed for submission at the NSS Municipal office.*
Both Subvented and Non-subvented categories.
NB;❗❗❗❗❗❗❗❗
1. Annual evaluation form is different from monthly evaluation form. That is what will guarantee your NSS certificate.
2. There are two options with regards to the form.

Kindly proceed with that if you don’t have any issue with your details.
🔰 *REQUEST CHANGES*
Kindly click on that if you have problems with your name ie. Spelling mistake, arrangement of names etc.

3. Submission officially starts from *Monday, 21st June 2021*
4. After your form has been endorsed by your supervisor, kindly make sure you run a copy for yourself before you submit the original.
5. Submission is at the NSS municipal office, ministries.
